WHAT IS SUPPORT PLANNING?

 
Support Planning is a person-centric process, empowering the individual to make their own decisions on the support and providers they require to enhance their lives. The resulting Assessments Australia report is a support plan detailing the services required by the individual of a formal and informal nature, which can be supported by an individual budget.
 

HOW IS THE SUPPORT PLANNING PROCESS UNDERTAKEN?

 
Following an initial pre-session phone interview to collect necessary, essential information your Assessments Australia professional meets face to face with the individual to consider all available options, aspirations and requests. Following these interviews a an individual support plan is developed.
