Relative and Kinship Care Assessments

Status: In Progress (March 2008 - March 2011)

Assessments Australia has been appointed as a Preferred Provider to perform assessments to determine the suitability of carers to undertake relative/kinship care, and their eligibility for the Supported Care Allowance. These assessments are performed throughout NSW at the request of the Department, with Assessments Australia now having completed over 1000 assessments.

High Level in home Personal Care Needs: Assessments

Status: Currently Renegotiating (March 2008 - June 2010)

Assessments Australia has been contracted to perform assessments of personal care needs for individuals with a physical disability and high support needs, including promoting ongoing improvements in the quality and consistency of the assessment process.

High Level in home Personal Care Needs: Review

Status: Completed (2009)

Assessments Australia was contracted to review the format of the High Level Personal Care Needs Assessment (HLPCNA) and the associated usage guidelines. These modifications were tested in the field with reports presented to ACPDU.
This contract was to help inform the Department of ways to improve efficiency in use of this assessment tool and to improve the consistency in collection and reporting of assessment results.

Respite, Assessment and Booking System

Status: In Progress (Since 2008)

Assessments Australia was contracted by ADHC to review Centre-Based Respite needs assessment and prioritisation practices and provide recommendations for a new system implementation. A prototype Online Booking System has been designed and trailed as part of this project, with a statewide rollout of the recommended framework and booking system to occur shortly. The new system will involve a structured prioritisation approach based upon the Support Needs Assessment and an assessment of carer need as well as the introduction of the Care Load Index which will provide guidance in client allocation to Centre-Based Respite Centres.

Assessing Client Support Needs in Large Residential Centres

Status: Completed (Jan 2007)

In July 2006, Assessments Australia assessed the support needs of clients in Large Residential Centres across New South Wales with over 1200 assessments being completed in a 4 week period. These profiles are being used to support the development of service planning options and models, particularly for those residents with complex health care needs and behaviour management issues.

Costing and Classification Study: Assessments

Status: Completed

The Costing and Classification Study was conducted by the University of Wollongong on behalf of Ageing, Disability and Home Care (ADHC). Assessments Australia was contracted by ADHC to conduct the assessments based on the HACC Screening Tool. Clients were assessed to identify their functional capabilities. Assessors were trained in the use of the tool and conducted just under 1600 assessments in 5 weeks. All quality assurance and appeals were managed in the same time frame.

Community Access

Status: Ongoing (Since 2004)

Assessments Australia performs assessments on school age applicants to determine whether they are eligible for one of ADHC’s two Post School Programs: Transition to Work or Community Participation. The assessment also determines the funding level which will be allocated to a service provider where an applicant is assessed as eligible for Community Participation. These assessments identify the functional capabilities of the applicant, as well as the level of behavioural intervention required to support the applicant. Funding band appeals are also completed as part of this project.

Group Homes: Support Needs Assessment

Status: Ongoing (Since 2003)

All clients in Group Homes are assessed to determine the level of staff support required in the Group setting. This level of support forms part of the mechanism for the allocation of funds to a Group Home. Support Needs Assessments are ongoing as they are incorporated into the Group Home budget allocation process.

Group Home Roster Analysis

Status: Completed

In 2003, roster patterns in ADHC Group Homes were examined to determine if there had been any substantive changes over a three-year period. Findings of the project were then referenced to a series of financial benchmarks related to client unit costs and house level benchmarks based on client support needs.
